Insurance

NEW: February 2020 - Now accepting Fidelis Care and Medicaid Managed Care Plans. Unfortunately, we do not accept traditional Medicaid programs.

"Will my insurance cover my therapy appointments?" That's a question we hear a lot. In almost every case, we can proudly answer, "we participate with nearly all insurance plans in New York State. As every plan is different, many have co-pay, co-insurance or deductible obligations as a part of the particular plan.

We get your insurance details during our first communication to verify your benefits before your evaluation appointment. Your co-pay, co-insurance or deductible payment obligations will be reviewed in a patient meeting at your first appointment. Insurance payments are due at time of service. All major credit cards, debit, cash or checks made payable to CPT are accepted.

We can also obtain authorization if that is required by your insurance plan.

Did you know that you can be evaluated and treated by a licensed Physical Therapist without a referral/prescription from your physician? This is referred to as the “No Prescription – No Problem Program” (Direct Access) which permits 10 visits or 30 days, whichever comes first. Many insurance plans allow physical therapy services using this program. Some do not.

We gladly accept worker's compensation and motor vehicle claims. We are proud to work with the Department of Veterans Affairs.

If you do not have insurance or choose to not use your insurance benefits, we offer a self-pay rate. Please contact us for more information.

A referral from your physician is required if your insurance plan is:

  • Traditional Medicare
  • Fidelis Care Plans
  • Medicaid Managed Care Plans
  • Worker's Compensation
  • Motor Vehicle Claims

We can assist in getting a physician referral, verify your coverage and review your options with you.

We gladly accept worker's compensation and motor vehicle claims. We are proud to work with the Department of Veterans Affairs to serve the needs of our valued veterans in our community.
